Surgical Recovery Program
There are a lot of things to think about when preparing for major surgery. What are the risks? Will you be in pain? Have you thought about what help you’ll need to recover? Will you be able to stay at home while you heal? Although every surgical recovery is different, the goals of home-based clinical care post-surgery include patient and family involvement, shared decision-making, and ﬂexibility around changes in plans to meet your needs. Having home care services can help you recover more quickly, keep you safe at home, and just as importantly, help you avoid another hospital stay.

C&C Healthcare Services is also involved in developing innovative ways to ensure that key information about the health of older adults is available to those who need it. We assist in planning programs, set priorities, and keep track of the latest trends. In response to the growing need for supportive communities that enhance mobility, we stay on top of reports highlighting the efforts of the Atlanta Regional Commission’s (ARC’s) Area Agency on Aging.
C&C is creating a plan for the healthcare of its clients with its “Care of Life” Programs. C&C will make an effort to keep up with
agencies which highlights the need to maintain the progress made on several health indicators and increase our efforts to address other important health issues.
Creative Meals Program
C&C will encourage families and staff to eating fruits and vegetables. Daily diets rich in fruits and vegetables may reduce the risk of some cancers and chronic diseases, such as diabetes and cardiovascular disease. Fruits and vegetables provide essential vitamins and minerals, fiber, and other substances that are important for good health.
A greater proportion of Obesity is defined as having a body mass index (BMI) of 30 or higher. BMI is calculated by dividing a person’s weight in kilograms by his or her height in meters squared (kg/m2). The BRFSS uses self-reported data about height and weight, which may lead to under estimating obesity in the United States. The National Health and Nutrition Examination Survey (NHANES), which takes body measurements, estimates the prevalence of obesity among older adults at 34.6%.18
Older adults can benefit from maintaining a healthy body weight. Obesity is a risk factor for many chronic conditions, including stroke, heart disease, cancer and arthritis.
The environment plays a role in helping to fight obesity. People may make decisions on the basis of their environment or community. For example, a person may choose not to walk to the store or to work because of a lack of sidewalks. Communities, homes, and workplaces can all influence people’s health decisions. Because of this influence, it is important to create environments in these locations that make it easier to be physically active and eat a healthy diet. Adults aged 65 years or older eat 5 or more fruits and vegetables daily compared with other age groups.

Sometimes the details of life can become overwhelming. When this happens, our Care Concierge Program are available to handle them. Whether assisting with mail, helping to pay bills, or coordinating meal delivery, our care concierge services allow seniors to remain independent in their own home as long as possible.
Check In Program
C&C staff does a weekly “Check-In” service to ensure you or your loved one is staying healthy and safe by our providing care givers. We “Check-in” making sure we are doing our job with many of the daily living activities whether it’s a few hours to 24-hours a day, or 7 days a week.
Physical Activity Program
strengthening. Strong evidence shows that regular physical activity is safe and reduces the risk of falls among older adults. Older adults at risk of falling should do exercises that maintain or improve their balance. For best results, they should do these exercises at least 3 days a week from a program shown to reduce falls.For Example: Activity required:
2 hours and 30 minutes (150 minutes) of moderate-intensity aerobic activity (i.e., brisk walking) every week and muscle-strengthening activities on 2 or more days a week that work all major muscle groups (legs, hips, back, abdomen, chest,shoulders, and arms).
or 1 hour and 15 minutes (75 minutes) of vigorous-intensity aerobic activity (i.e.,jogging or running) every week and muscle-strengthening activities on 2 or more days a week that work all major muscle groups legs, hips, back, abdomen, chest, shoulders, and arms).
or an equivalent mix of moderate- and vigorous-intensity aerobic activity and muscle-strengthening activities on two more days of the week on major muscle groups.
Mobility is fundamental to everyday life and central to an understanding of health and well-being among older populations. Impaired
mobility is associated with a variety of adverse health outcomes. As the age of the U.S. population continues to increase, aging and public health professionals have a role to play in improving mobility for older adults. There are critical gaps in the assessment and measurement of mobility among older adults who live in the community, particularly those who have physical disabilities or cognitive impairments. By changing physical environments and creating unique integrated interventions across various disciplines, we can improve mobility for older adults.
